# Custom AI receptionist for clinics: build or buy

Most clinics should buy. Here are the three cases where building your own wins, what each option costs, and what a build involves.

Most clinics should buy an AI receptionist. A custom AI receptionist pays off only when the bought ones can’t book into your system, speak your patients’ languages or follow your routing rules. Bought tools charge around £1 a call or £349 a month per site; building means owning telephony, booking integrations, safety rules and compliance yourself.

The market has filled up fast. Our roundup of the [best AI receptionists for clinics](https://zeroic.in/blog/best-ai-receptionists-for-clinics) covers twelve tools across six countries, and most single-site clinics will find one that fits. The build question comes up with clinic groups, hospitals and healthtech companies, where the receptionist has to work inside systems and rules no vendor has seen.

## Buy first unless you fail three tests

Run your clinic through three questions before you talk to a developer:

1. **Does a vendor book straight into your booking system?** Off-the-shelf tools integrate with the common practice systems in their market. Zocdoc’s [Zo phone assistant](https://www.zocdoc.com/business/ai-phone-assistant) books “directly into your EHR”. If your scheduler is an in-house hospital system or a custom app, no vendor will be on its integration list.
2. **Does it speak the way your patients speak?** Language lists look long on vendor pages. What matters is whether the agent handles your callers’ mix, such as Hindi and English in the same sentence, or a regional accent, on a phone line.
3. **Can it follow your rules?** Which doctor sees which complaint, which slots are held back for follow-ups, which calls always go to a nurse. Bought tools let you configure common rules. They won’t let you write new ones.

If you pass all three, buy. You’ll be live in days, the vendor carries the maintenance, and you can switch if it disappoints. If you fail one, keep reading.

## What off-the-shelf AI receptionists cost

Bought tools price per location, per call or per minute, and the model tells you who they’re built for:

ToolMarketPrice (vendor’s own page)Booking integration

ViveoAIUK and Ireland dentalFrom £349/month per location, 500 minutes, then £0.70/minuteDentally and Aerona; new systems quoted separately

InTouchNowNHS GP surgeries£1.00 per call on Starter, £0.90 on All Calls; handbacks to reception freeBooks doctor and nurse appointments

AarvoxIndian hospitals₹5.5/minute, falling to ₹4.75 at full scaleNone: you forward your existing line

Zo by ZocdocUS practicesOn requestBooks into your EHR

Two details stand out. [InTouchNow](https://intouchnow.ai/) doesn’t charge for calls passed back to reception, and lists itself as a Class I medical device (UKCA) compliant with DCB 0129 and DCB 0160. That regulatory work is part of what you’re paying for, and a custom build in the NHS has to do the same. [Aarvox](https://www.aarvox.in) works by forwarding your line with “no PMS integration”, which makes it fast to switch on. Check where its bookings land before you sign.

[ViveoAI](https://viveoai.com/pricing) runs month to month with a 14-day pilot. That’s the pattern to look for in any bought tool: short commitments, so you can leave if call quality is poor.

## When a custom AI receptionist pays off

Building wins in three situations, and each one maps to a failed test above.

**Your booking system is yours.** Hospital groups often run their own scheduling, or a hospital information system with a thin API. A bought receptionist that can’t see the diary takes messages. One that can check slots, hold them and confirm by SMS completes the booking. The integration is the product here, and it’s the part vendors won’t build for one customer.

**Your patients need a language mix vendors don’t test.** Speech recognition accuracy varies by language and by audio quality. Our guide to [AI voice agent architecture](https://zeroic.in/blog/ai-voice-agent-architecture) explains why every language needs its own evaluation on recorded calls. If your callers switch languages mid-sentence, you want to choose the speech models per language, which bought tools don’t let you do.

**The receptionist is part of your product.** If you’re a healthtech company selling scheduling, telehealth or patient engagement, a white-labeled voice agent inside your own app is a feature you own. Our breakdown of the [cost to build a telehealth app](https://zeroic.in/blog/telehealth-app-development-cost) covers the layers around it.

Volume alone rarely justifies a build. A group with 20 sites on a common booking system is better served by a vendor’s group plan than by its own engineering team.

## What building one involves

A custom build has five layers. You can rent the first two and build the rest.

### Telephony and the voice pipeline

The phone number, speech-to-text, the language model and text-to-speech. A voice platform runs this for a per-minute fee: [Retell’s pricing page](https://www.retellai.com/pricing) lists $0.07-$0.31 a minute for voice agents on pay-as-you-go, with 20 concurrent calls included and more at $8 per concurrent call per month. At 3,000 call minutes a month, that’s $210-$930 in usage before the engineering. If you outgrow the platform, [Pipecat](https://github.com/pipecat-ai/pipecat) is an open-source, BSD-licensed framework with serializers for Twilio, Telnyx, Plivo, Vonage and Exotel, so you can run the pipeline yourself. Carrier costs are small either way: [Twilio lists US local numbers](https://www.twilio.com/en-us/voice/pricing/us) at $1.15 a month and inbound calls at $0.0085 a minute.

### Booking integration

This is where most of the build time goes. The agent needs tools to find a patient, read open slots by doctor and location, hold a slot, book, cancel and send a confirmation. Every tool returns a structured error, and the agent has a scripted line for each: “the system didn’t confirm, so the front desk will text you within the hour” beats a made-up confirmation.

### Routing and escalation rules

Which calls go to a person, when, and with what summary. In hours, a warm transfer with the caller’s details. Out of hours, a logged callback or the on-call number. These rules are clinic-specific, which is why they’re the main reason to build.

### Safety rules

The agent books and answers practical questions. It doesn’t give clinical advice. [ECRI ranked misuse of AI chatbots](https://home.ecri.org/blogs/ecri-news/misuse-of-ai-chatbots-tops-annual-list-of-health-technology-hazards) as the top health technology hazard for 2026, noting the tools “are not regulated as medical devices nor validated for healthcare purposes”. For a receptionist, that means a fixed list of symptom and emergency phrases that end the AI conversation and route the caller, tested on recorded calls before launch.

### Compliance

In the US, every vendor that touches patient data needs a business associate agreement. [Twilio signs one](https://www.twilio.com/en-us/hipaa) only for Security Edition or Enterprise Edition customers. Retell’s pricing page lists a custom BAA under its enterprise plan. If call summaries land in Supabase, [HIPAA projects need](https://supabase.com/docs/guides/platform/hipaa-projects) a signed BAA, the HIPAA add-on and point-in-time recovery. The UK and India have their own clinical safety and data protection rules, and they need the same attention up front.

## Build or buy: the decision checklist

- **One site, common practice system, English-speaking callers:** buy. Pick a tool with a monthly contract and a pilot.
- **NHS GP surgery:** buy. Vendors like InTouchNow have already done the medical device and DCB work.
- **Hospital or clinic group with its own scheduling system:** build the booking integration and routing; rent the voice pipeline from a platform.
- **Callers who mix languages in one call:** build, so you can test and pick speech models per language.
- **Healthtech product adding a voice receptionist:** build. It’s a feature of your product, and you’ll change it every month.
- **Unsure which side you’re on:** pilot a bought tool for two weeks and log every call it fails. Those failures are your build spec.

## How Liya AI shipped voice coaching in 6 weeks

Our closest named project is [Liya AI](https://zeroic.in/liya-ai), an AI companion with chat and voice coaching for employees, which our team built in 6 weeks. We built the chat and voice coaching pipelines end to end, and a voice agent that listens, responds and keeps context across a session faces the same pipeline problems a receptionist does. Liya’s privacy-first insights layer shows managers aggregated signals without exposing individual conversations, the same discipline a clinic needs around patient calls.

If your clinic fails one of the three tests, we scope a first version, one site, one booking system and one or two languages, as a fixed-price build through our [AI development company](https://zeroic.in/ai-development-company) work. If you’d rather know which side you’re on first, an [AI audit](https://zeroic.in/services/ai-audit) maps your call types and systems and gives you the answer in 1-2 weeks.

## Frequently asked questions

**How much does an AI receptionist for a clinic cost?**

Bought tools are priced per location, per call or per minute. ViveoAI starts at £349 a month per location with 500 minutes, InTouchNow charges £0.90-£1.00 per call, and Aarvox charges ₹5.5 a minute. Building your own swaps that fee for usage costs, such as Retell's $0.07-$0.31 a minute, plus the engineering to build and maintain it.

**Can I build a custom AI receptionist on a voice platform instead of from scratch?**

Yes, and for most first versions you should. A voice platform such as Retell runs the speech and telephony layers for a per-minute fee, so your engineering goes into booking integrations, routing rules and safety checks. Move to an open-source framework like Pipecat when you need full control of the pipeline, your own hosting or lower per-minute costs at volume.

**Is an AI receptionist HIPAA compliant?**

Only if every vendor that touches patient data signs a business associate agreement and is configured for it. That covers the phone carrier, the voice platform, the model provider and the database. Twilio signs a BAA only on its Security or Enterprise Edition, and Supabase requires a signed BAA plus its HIPAA add-on. A vendor saying it is compliant is not enough.

**Should an AI receptionist give medical advice to patients?**

No. A clinic receptionist agent should book, reschedule, answer practical questions and route calls, and hand anything clinical to a person. ECRI ranked misuse of AI chatbots as the top health technology hazard for 2026 because these tools are not validated for healthcare. Set fixed escalation rules for symptoms and emergencies, and test them before go-live.

**How long does it take to build a custom AI receptionist for a clinic?**

A first version for one site, one booking system and one or two languages fits a fixed-scope build of about 5-6 weeks. Each extra booking system, language or site adds integration and testing work. Plan for a pilot on live calls with a human backup line before you route all traffic to the agent.
